Every year, all schools are required to report the immunization status of their kindergarten and seventh grade students, and twelfth grade students according to G.S. 130A-155(c). According to G.S. 130A-440, beginning with the 2021-2022 school year all children enrolling in the public schools for the first time are required to submit proof of a health assessment.
The purpose of these reports is to ensure all children enrolled in North Carolina schools are protected against vaccine-preventable diseases and to determine the number of children who meet state immunization and health requirements.
