Vaccine Information Statements (VISs)

Federal law requires all vaccine providers make VISs available. Providers must:

  • give the appropriate VIS to the recipient or to the recipient’s parent or legal representative with each dose of vaccine,
  • give the appropriate VIS prior to administration of the vaccine,
  • give the appropriate VIS each time the vaccine is given (not just with the first dose), and
  • record certain information in the patient’s permanent medical record.

Vaccine providers should ensure their VISs are current. Check the dates on your stock of VISs against the dates on this list. If you have outdated VISs, discard them and print current versions. VISs in additional languages and formats (including audio formats and formats suitable for download to a mobile device) are available through the CDC website. To ensure you know when a VIS is updated, sign up for email updates. Up-to-date news on VISs and pending edits are listed on the CDC’s News about VISs page.
