North Carolina Immunization Program (NCIP) Requirements

Vaccine Information Statements (VISs)

Federal law requires all vaccine providers to make VISs available. A provider’s responsibility includes:

  • With each vaccine dose, provide the appropriate VIS to the patient, or their parent or legal representative, before and each time it is given (not just the first dose).
  • Record certain information in the patient’s permanent medical record.

Vaccine providers should ensure their VISs are current. Check the dates on your stock of VISs against the dates on this list. If you have outdated VISs, discard them and print current versions.
